A printed duvet cover is flat. Light hits it evenly and it reads as flat, however good the pattern is.
Tufting is raised off the surface, so each row throws a small shadow. That shadow is the whole difference — it is why a tufted bed looks layered and considered in a photograph, and why a flat one never quite does.
The diamond-and-stripe pattern here is worked row by row into a soft woven base, and it stays raised through the wash rather than flattening out after a month.
